NurScopeTM was developed by nurses, through a collaboration between NANDA-I and the University of Wisconsin - Green Bay (USA). The e-tool is designed to base nursing diagnosis on nursing assessment, nursing knowledge and clinical judgment - rather than linking nursing diagnosis to medical diagnoses / conditions.

All 235 diagnoses in NANDA International Nursing Diagnoses: Definitions and Classification 2015-2017 are included in NurScopeTM!


Identify priority nursing diagnoses by following three steps:
  1. Select the e-tool appropriate diagnostic indicators, based on nursing assessment and other patient data;

  2. When presented with a list of potential diagnoses, review the details of each to refine, validate and prioritize the most accurate diagnosis(es).

  3. Document your decisions by emailing a summary of your work to your professor.
  • Clinicians can use NurScopeTM to ensure individualized care, by identifying relevant nursing diagnoses which are foundational to patient-specific plans of care. Standardized assessment tools, such as the Morse Fall Scale, are integrated into the e-tool and easily accessed. HIPA compliance is assured as no patient identifiers are used in the e-tool.
  • Nursing educators will find NurScopeTM to be a versatile teaching resource, as its powerful decision support design demonstrates and develops clinical reasoning skills. The e-tool’s emailed summary provides data to evaluate student proficiency in nursing assessment, diagnosis and clinical reasoning, NurScopeTM is easily deployed in a variety of educational settings, including classroom demonstration, small group case study, simulation lab scenarios and clinical assignments.

  • Students can use NurScopeTM to efficiently and accurately complete case study, simulation and clinical assignments. The e-tool demonstrates how to link assessment data to relevant nursing diagnoses and identify any additional data needed. Integrated standardized scales and tips for diagnosis differentiation help confirm diagnostic decisions. The emailed work summary saves time in assignment documentation.
NurScope provides four content access points – or pathways – which have powerful clinical reasoning and time-saving features. All pathways lead to detailed nursing diagnoses, however choosing a specific pathway helps customize use of the e-tool to meet priority information needs. For example:
  • The Domain pathway will be a good starting point for those accustomed to using the NANDA-I taxonomic structure of domains and classes.

  • The Diagnostic Focus pathway works well when you have determined a patient problem and need to identify potential nursing diagnoses.

  • The Nursing Diagnosis pathway is suggested for those who want to validate an initial diagnosis, or might be looking for additional information about a familiar diagnosis.

  • The Assessment pathway (coming in a later release) begins with patient assessment using Gordon's functional health patterns and selection of appropriate diagnostic indicators (defining characteristics and risk factors) for the patient.
The integrated Multimedia Learning Library provides related resources e.g, the Morse Fall Scale for use in assessing risk for falls, with results linked directly to associated defining characteristics. Videos provide information on nursing diagnosis, the process of assessment to diagnosis, and Frequently Asked Questions.

Differential and possible co-occurring nursing diagnoses are provided, enabling review and cross-check to validate final diagnosis.

Critical assessment questions, recommended standardized screening tools, and expert recommendations are provided to clearly identify potential patient safety issues that may require immediate attention.
